Piperacillin 4g + Tazobactam 0.5g Injection is an antibiotic belonging to the Extended-spectrum penicillins group in beta lactams category.

It is also known as an antipseudomonal penicillin as it is effective in Pseudomonas aeruginosa.

It is a combination drug piperacillin is only available with β-lactamase inhibitor tazobactam. Tazobactam is required to extend the activity of piperacillin against β-lactamase-producing strains of S aureus and Gram-negative bacteria.

It is used clinically inStreptococcal, meningococcal and neurosyphilis infections.

Mechanism of action:It acts by preventing bacterial cell wall synthesis by binding and inhibiting cell wall transpeptidases leading to death of the bacterial cell.

It is available as Tazocin&Zosyn  in both International and Zosyn Indian markets.

 

Uses

  • Piperacillin 4g + Tazobactam 0.5g Injection is used in Intra-abdominal caused by Bacteroides fragilis and  Escherichia coli
  • It is used in Female pelvic infections caused by Escherichia coli .
  • Zosyn is used in skin infections and diabetic foot caused by Staphylococcus aureus.
  • Tazocin is also prescribed in Community-acquired pneumonia caused by Haemophilusinfluenzae and Nosocomial pneumonia caused by Staphylococcus aureus,Haemophilus influenzae, Klebsiella pneumoniae and Pseudomonas aeruginosa.

 

Dosage

  • Piperacillin 4g + Tazobactam 0.5g Injection comes as white to off-white powder in vials for reconstitution with the solvents and is further diluted with solutions as per instructions.
  • It is given as intravenous infusion over 30 minutes or intramuscularly.
  • The usual daily dose of Zosyn for adults is 12.0 g piperacillin/1.5 g tazobactam divided in 6 hours.
  • Final dosages are determined by the doctor on the basis of factors such as pre existing medical condition, severity of infection, age, weight and ongoing medications.
  • Dose and rate of infusion is adjusted to minimise related side effects.

Precautions

  • Piperacillin 4g + Tazobactam 0.5g Injection is contraindicated in cases of allergy or history of allergic reactions to any of the penicillins, cephalosporins, or β-lactamase inhibitors.
  • Patients must be monitored carefully for severe allergic reactions and diarrhea related to antibiotics.
  • Regular tests for kidney and blood are necessary.
  • Tell the doctor about pre existing medical conditions, ongoing medications and any allergy before starting the drug.
