Isosorbide Dinitrate 5mg, 10mg tablets are vasodilators belonging to the nitrates group.

It is highly lipid soluble which ensures rapid absorption sublingually.

It is an organic dinitrate which further metabolized into two mononitrates both having vasodilation properties.

Vasodilators reduce preload or afterload or both of the blood supplied to the heart.

It also reduces damaging remodeling of the heart.

Mechanism of action:it acts by releasing nitric oxide which causes relaxation of smooth muscles of blood vessels by activating guanylyl cyclase and increases cGMP resulting in decrease of venous return and size of heart. It also increases coronary blood flow.

It is available as Sorbitrate in both International and Indian markets.

 

Uses

  • Isosorbide Dinitrate 5mg, 10mg tablets are used sublingually for acute chest pain in coronary artery disease and orally as prophylaxis for the same.
  • It is also used in congestive heart failure as a combination of hydralazine and isosorbide dinitrate as isosorbide dinitrate/hydralazine (BiDil) which dilates arterioles and veins simultaneously.

 

Dosage

  • Isosorbide Dinitrate 5mg, 10mg tablets are taken sublingually or orally.
  • On experiencing angina one tablet is placed under the tongue of the patient till it dissolves.
  • Sorbitrate is taken orally with water daily as prophylaxis treatment.
  • Sublingual doses are smaller than oral doses as sublingually it reaches therapeutic plasma concentrations rapidly after absorption.
  • Do not stop taking the Sorbitrate without consulting the doctor.
  • Final doses are determined by the doctor on the basis of pre existing medical condition, ongoing medications, age and any allergy.
  • Doses are also modified according to dose related side effects and response to the drug.
  • It is stored at temperature below 250C away from moisture in its original container.

Precautions

  • Sorbitrate is contraindicated in case of allergy to nitrates.
  • Close monitoring is required in cases of low blood pressure, severe anemia, congestive heart failure and kidney dysfunction.
  • Isosorbide Dinitrate 5mg, 10mg tablets interact with phosphodiesterase type 5 inhibitors resulting in synergistic hypotension.
  • In the beginning of the treatment medication for headaches is required.
  • Alcohol must be avoided while taking this drug.
  • This drug causes dizziness, so driving and use of machines must be avoided.
  • Always tell the doctor about pre-existing medical conditions, ongoing medications, any history of allergic reaction, pregnancy , future plan of pregnancy and pregnancy.

It must not be used in pregnancy and breastfeeding mothers.
