Infliximab 100mg Injection is an immunosuppressant agent that belongs to the TNFalpha blockers group.

It is a chimeric (25% mouse, 75% human) IgG1 monoclonal antibody.

Its efficacy reduces after formation of anti Infliximab antibodies in the body, so it is not recommended for monotherapy.

Mechanism of action:It acts by binding to specifically  soluble and membrane bound TNF-α preventing its interaction with p55 and p75 cell surface receptors. This leads to down-regulation of macrophage and T-cell function leading to decreased immunity of the body.

It is available as Remicade in both International and Indian markets.

 

Uses

  • Infliximab 100mg Injection is indicated in Crohn’s Disease ,Pediatric Crohn’s Disease, Ulcerative Colitis, Pediatric Ulcerative Colitis, Ankylosing Spondylitis and Plaque Psoriasis Psoriatic Arthritis.
  • Remicade is used with methotrexate in the treatment of Rheumatoid Arthritis.
  • Other indications of Remicade are granulomatosis with polyangiitis (Wegener’s granulomatosis), giant cell arteritis, Behçet’s disease, uveitis,and sarcoidosis.

 

Dosage

  • Infliximab 100mg Injection comes as a vial containing 100 mg lyophilized powder to be reconstituted with20 ml vial before using as injection and further diluted for infusion intravenously.
  • Unused portion of the solution must be discarded as it contains no antibacterial preservatives.
  • The usual dose of Remicade is 3–5 mg/kg every 8 weeks.
  • Final doses are determined by the doctor on the basis of pre existing medical condition , ongoing medications, age, weight and any allergy.
  • Doses and infusion rate are also modified according to dose related side effects.

Precautions

  • Infliximab 100mg Injection is contraindicated in cases of hypersensitivity to infliximab and moderate to severe heart failure.
  • Close monitoring is required for signs and symptoms of serious infections, hepatotoxicity, malignancies, latent tuberculosis , reactivation HBV, heart failure, hematologic, neurologic and allergic reactions during and after treatment.
  • The combination of infliximab and anakinra or abatacept or tocilizumab is not recommended because of high risk of serious infections.
  • Live vaccines must not be given concurrently with infliximab.
  • Always tell the doctor about pre-existing medical conditions, ongoing medications, any history of allergic reaction, pregnancy , future plan of pregnancy and pregnancy.

It must be used in pregnancy and breastfeeding mothers with caution.
