Drug Name: Tenecteplase 40mg Injection
Details
Tenecteplase 40mg Injection is a blood affecting drug which belongs to a group of fibrinolytic or thrombolytics and sub grouped as tissue plasminogen activators (t-PAs).
It is a mutant human recombinant t-PA and produced in a Chinese hamster ovary cell line.
It is a prescription only drug.
Fibrinolytics are used to break the clot blocking blood vessels leading to various complications.
Mechanism of action:It is the fibrin specific which means it acts on the plasminogen attached to the fibrin in the clot and has very little effect on the circulating plasminogen.Then this plasminogen generates serine protease Plasmin which lyses the fibrin leading to the breakdown of clot.
It is available as Metalyse In both Indian and International markets.
Uses
Tenecteplase 40mg Injection is indicated for use in the acute myocardial infarction (AMI) to reduce the mortality and must be given as soon as after the onset of AMI.
Dosage
- Tenecteplase 40mg Injection comes as a single use powder vial and is mixed with the solvent provided before injecting.
- Metalyse is directly administered into the blood circulation by intravenous administration in a single bolus over 5 seconds only by healthcare professionals only.
- Dosages are determined by the doctor according to the severity of stroke, weight, any allergy, ongoing medications and pre existing medical conditions.
- Metalyse is stored in the refrigerator (2 – 8°C) but do not freeze.
- After reconstitution it is stored at 2−8°C (36−46°F) and must be used within 8 hours.
Side effects
- Hemorrhagic bleeding which may lead to collapse, weakness of the arms or legs, bruising of skin, blood in urine.
- Lightheadedness
- Fever
- Dizziness
- Headache
- Nausea and vomiting
- Bleeding and bruising at the site of injection
- Hypersensitivity
- Cardiogenic shock
- Pulmonary edema
- Hypotension
- Pericarditis
Precautions
- Tenecteplase 40mg Injection is contraindicated in active internal bleeding, cerebrovascular accident history, last 2 months history of Intracranial or intraspinal surgery or trauma, Intracranial neoplasm, arteriovenous malformation, or aneurysm and severe uncontrolled hypertension.
- Extra care is required to avoid any injury or bruising which might lead to bleeding complications.
- Tell the doctor about any medical condition, ongoing medications (like aspirin or ace inhibitors), allergy, pregnancy or future plan of pregnancy and breastfeeding.
- Careful monitoring of patients is required for the signs of hypersensitivity and other drug interactions.
- Always tell the doctor about pre-existing medical conditions, ongoing medications, any history of allergic reaction, pregnancy, future plan of pregnancy and pregnancy.
Metalyse must not be used in pregnant and breastfeeding women.
